Write questions about OBSERVABLE behaviour, not self-rating. "How mature is your
process, 1 to 5" measures confidence; "when someone leaves, how long until their
access is revoked" measures the thing.

The result must be genuinely useful even to someone who never contacts you.
A score with no explanation is a lead magnet that leaves a bad taste; a score with
the two weakest areas named and one concrete action each is worth their email.
