Split by SOURCE. A blended pass rate averages referrals against an unfiltered
application flood, and every decision made on it is wrong: agencies look bad, job
boards look fine, and the channel that actually produces hires is invisible.

Measure time in stage, not just total time to hire. Candidates are lost while
waiting, and the waiting is almost always in one stage, usually between the
interview and the decision.
Compute pass rates per stage per role. An interview stage that passes 95% is not a
stage, and one that passes 10% is either broken or in the wrong order.
