Run an 8D

Containment, root cause and verified corrective action, in the customer's format.

Fill it in

What failed, where, and what the customer saw.

Before analysis, because parts are still shipping.

With data, not with a statement of intent.

Your prompt

Run the 8D.

[the escape]

Contain FIRST and say what you did, with dates and quantities. Parts are still
moving while the analysis happens, and an 8D that arrives with a beautiful root
cause and no containment is a report the customer reads angrily.
Find the root cause with evidence, and answer both questions: why it happened, and
why it escaped. Those are different failures with different fixes, and reports that
answer only the first are why the same escape recurs.

Verify the corrective action with DATA from after it was implemented. "Training
completed" is not verification, and neither is a statement of intent.
Say where else this could apply. That section is what turns one 8D into prevention.
Use Run an 8DOpens with everything above already filled in.

Why this works

An 8D that arrives with a beautiful root cause and no containment is read angrily, because parts were still moving. This contains first, answers both why it happened and why it escaped, and verifies with data from after the fix.
