What each plan actually costs a person, given how they use care.

Model actual USAGE scenarios rather than comparing premiums. The plan with the
lowest premium is the most expensive one for anybody who uses care, and the
comparison table everybody gets does not show that.
Compute total annual cost per scenario: premium plus deductible plus coinsurance up
to the out-of-pocket maximum, and include the employer contribution to a savings
account where there is one, because that is real money most people ignore.
